Traumatic injury to the hand may result in a subcutaneous tissue loss. Exposed tendon, bone, or joint surface is best treated by flap coverage. The reversed cross finger subcutaneous tissue flap from the dorsum of one finger can be widely applied to hand surgery. We experienced a case of nonunion with bone and subcutaneous defect on middle phalanx of the index finger. The patient was treated by illiac bone graft with K-wires fixation and reversed cross-finger subcutaneous tissue flap. We report a case and review related articles briefly.
